摘要
We explore the consequences of physicians’ excessive workload from the perspective of decision fatigue—the cognitive depletion due to an increased number of patients and numbers of decisions. Leveraging an administrative dataset of over 250,000 emergency department visits, we find that as physicians treat more patients and make more decisions, they reduce admissions, order fewer tests, and shorten hospital stays. These behavioral shifts lead to higher patient revisit rates, indicating declines in care quality. These effects are most pronounced for low-acuity cases, while for high-stakes patients physicians increase admissions—a protective response to cognitive strain. Our findings indicate that effective fatigue management goes beyond simply limiting shift durations, and must address both cognitive load and structural workforce constraints.

叶晗，香港岭南大学经济学系助理教授，2020年获新加坡国立大学经济学博士学位。主要研究领域为健康经济学、行为经济学和劳动经济学。研究成果发表于Review of Economic Studies, Review of Economics and Statistics, Journal of Human Resources, AEA Papers and Proceedings,《管理世界》等国内外学术期刊。
